New Passport
Obtaining a New Passport Book and/or Card
First-time applicants of any age (infants included), applying for either the Passport Book or Passport Card, must appear in person with appropriate documentation as follows:
PROOF OF U.S. CITIZENSHIP
For Persons Born Inside the U.S.:
- A birth certificate issued by Vital Statistics with a raised seal, stating the applicant’s given name/surname, date, place of birth, and date the birth certificate was recorded with the state.
Note: Children under the age of 16 require the long-form birth certificate, listing both parents’ names. If a child under age 16 is re-applying for a passport, the long-form birth certificate still is required, even upon submission of an old passport. For Persons Born Outside the U.S.:
- A naturalization certificate or certificate of citizenship.
- Proof relevant to your parents’ naturalization, your foreign birth certificate and your admission to the United States for permanent residence.
Note: Birth abroad to U.S. citizen parents must be substantiated through a consular report of birth and/or certification of birth certificate, or your foreign birth certificate, proof of parent/citizenship and affidavit of U.S. citizen parents showing all periods/places of residence and physical presence in the U.S. and abroad before your birth.
PROOF OF IDENTIFICATION
Applicant must provide at least one of the following:
- A valid, not temporary or expired, driver’s license (permit not acceptable)
Note: If the driver's license is not New Jersey issued, then a secondary ID is required (see next bullet item). - A permanent ID card or pass from a government agency, employer, or student I.D., health insurance card, or social security card
TWO RECENT 2X2 COLOR PHOTOGRAPHS
Two identical color photographs (2 inches x 2 inches) must be submitted with your application. For your convenience, and to ensure that all federal guidelines established for passport photos are met, the Clerk’s Office provides photo services on-site at the time of application, for a fee of $10.00.
PROCESSING FEES
Payment is made in the form of cash or check/money order, so applicants should plan accordingly. As of February 1, 2008, the costs associated with obtaining a U.S. Passport Book and/or U.S. Passport Card are as follows:

Note: All first-time applicant fees include a $25 agency execution fee. For first-time applicants applying for both the passport card & passport book together, the $25 execution fee is applied only once. Children age 15 and under now require both parents’ signatures on their application.

The Philadelphia, New York, and Norwalk offices no longer offer "Walk-In" same-day service for new or renewal passports. You MUST make an appointment in advance; personnel available 24/7 by calling 1-877-487-2778 for any of the above-mentioned offices. You must be traveling within 14 calendar days and have proof of departure date (tickets or itinerary). If you require SAME DAY SERVICE, you must call BEFORE 10 a.m.
